COVID 19 – Update – Dec. 15, 2020

For Patients: We have implemented pre-screening questions involving your health and recent exposure to the COVID 19 virus. We will ask all patient to complete a PATIENT INFORMATION FORM before you arrive for your appointment. Please return these when you come for your appointment. When you arrive for your appointment, we ask you to come alone or bring only one other person, we will be asking you to use hand sanitizer, and social distance whenever possible. A mask is required when you come to our clinic. Please wear a mask or one will be provided. We have shields on our equipment and when necessary, the optometrist will be wearing a shield for your protection. We will be checking your temperature upon arrival to our store.

Our staff will be wearing masks and social distancing whenever possible. In between contact with patients we will be washing our hands. We will ensure your safety by cleaning high touch surfaces, like door handles, pens, dispensing tables, chairs, equipment, eyeglass frames and washrooms on a continuous basis.
